Health and Safety Accident Investigation that finds root cause - not just blame
For operations managers and directors responsible for workplace safety. Structured, independent investigations that protect your team, satisfy the HSE, and prevent the same incident happening again.

What's included in this service
A formal health and safety accident investigation into any workplace incident, near miss, or dangerous occurrence. The process goes beyond what happened to uncover why it happened - examining equipment, training, environment, and working culture to identify the root cause.

Rapid response and evidence
Your investigator visits site to gather evidence while conditions and memories are fresh - ideally within 24 to 48 hours. Photographs, witness interviews, equipment checks, and environmental conditions are documented before anything changes.
Report and root cause analysis
Evidence analysed using HSG245 (HSE's Investigating Accidents and Incidents methodology) to identify systemic failures in processes, training, or controls - not just "human error." You get clear, factual documentation suitable for HSE, insurers, and legal proceedings, plus a prioritised action plan ranked by urgency. No 200-page document for the sake of it.
Post-investigation support
Leadership briefing in plain language, not just a filed report. Guidance on implementing corrective actions, updating risk assessments, and determining whether the incident is RIDDOR-reportable. If HSE liaison is needed, we handle that communication for you.

Sector-specific compliance
Builders Merchants
Merchant incidents like manual-handling injuries, falls from vehicles and forklift-pedestrian near misses rarely come down to simple carelessness. Our investigation looks at the layout, training and pressures behind the event to give you a root cause you can actually act on.

Warehousing & Logistics
Serious logistics incidents tend to cluster around MHE strikes, loading-bay falls and dock edges, often on thinly supervised shifts. Our investigation works through the equipment, segregation, training and shift culture to find out why it happened and how to stop the next one.

Manufacturing & Engineering
Machinery incidents usually trace back to guarding, isolation or production pressure rather than a single mistake. Our formal investigation works through the equipment, training and culture to find the real root cause, and gives you defensible evidence if the incident is reportable.

Garden Centres
Garden-centre incidents can involve members of the public, including children, as well as staff and forklifts. Our investigation looks across both public access and operations to find the systemic cause, so a single fix protects customers and staff together.

Metal Stockholders
The high-energy incidents in stockholding, such as load crush, lifting failures and cutting lacerations, rarely have a simple cause. Our investigation examines the lifting plan, the equipment and the working practices to find out where control was lost.

Childcare & Nurseries
A nursery incident, whether a child injury, a near miss or a safeguarding concern, needs an investigation that holds up with parents and regulators alike. Our process examines the environment, staffing ratios and supervision, and produces a record that stands up to Ofsted and the HSE.

Frequently asked questions
What does a Competent Person actually do?
A Competent Person takes legal responsibility for your H&S compliance. That covers risk assessments, audits, policy, training coordination and regulatory compliance. You get accountability for getting it done, not advice you have to action yourself.
Can you work alongside our existing H&S team?
Yes. Many clients use us to extend their internal capability - specialist assessments, regulatory liaison, and Competent Person cover during absences. We support your team, not replace them.
How quickly can you start?
One-off projects can typically be scheduled within days. Partnership clients have a named consultant operational within two to three weeks. Urgent situation - an HSE inspection notice, for example - we mobilise faster.
What makes your sector specialists different from generic consultants?
A generic consultant applies standard regulations. Our specialists know the specific hazards common in your industry and the enforcement priorities that apply to it. For builders merchants, that means forklift operations around public access. For childcare, it means Ofsted compliance alongside workplace safety. For warehousing, it means racking inspections and shift-pattern risk.
What's the difference between the Partnership, one-off projects, and Compliance Cloud?
The Partnership gives you a named Competent Person who manages compliance end-to-end. One-off projects deliver specific services without ongoing commitment. Compliance Cloud is the software platform for real-time compliance management. Many clients combine them.
