Rapid response and evidence
Your investigator visits site to gather evidence while conditions and memories are fresh - ideally within 24 to 48 hours. Photographs, witness interviews, equipment checks, and environmental conditions are documented before anything changes.
Report and root cause analysis
Evidence analysed using HSG245 (HSE's Investigating Accidents and Incidents methodology) to identify systemic failures in processes, training, or controls - not just "human error." You get clear, factual documentation suitable for HSE, insurers, and legal proceedings, plus a prioritised action plan ranked by urgency. No 200-page document for the sake of it.
Post-investigation support
Leadership briefing in plain language, not just a filed report. Guidance on implementing corrective actions, updating risk assessments, and determining whether the incident is RIDDOR-reportable. If HSE liaison is needed, we handle that communication for you.
