Rapid response and evidence
Your investigator visits site to gather evidence while conditions and memories are fresh - ideally within 24 to 48 hours. Photographs, witness interviews, equipment checks, and environmental conditions are documented before anything changes.

Evidence analysed using HSG245 (HSE's Investigating Accidents and Incidents methodology) to identify systemic failures in processes, training, or controls - not just "human error."
